How old are you? Why should you "know"?

Someone asks a child "How old are you?".
The child, being an orphan and in the streets for so long, answers "I do not know".
The answer surprises the asker.
But why?
What does really determined "how old" are we?
The fact that out ID card says so?
The fact that our parents told us so?
If there is no objective medical examination to show us how old we are, why do we insist on paying so much attention on that question?
